| IMPORTANT INFORMATION CONCERNING OUR PRACTICE PHILOSPHY MADISON AUDIOLOGY PRACTICE POLICIES Our goal is to provide you the best possible audiological care. To that extent, we have developed our practice policies as follows: (1) Minimum Waiting Time for Appointments. Generally, an appointment for a given procedure or consultation is given as soon as possible. In most instances, patients are seen within one weeks time. If required, patients are scheduled sooner than one week, and are often seen within one or two days depending on individual circumstances. Please make sure that all referral information is in order prior to your office visit. Referrals from your physician are necessary in many instances so please see to it that you have these forms available. In addition, previous test findings should be at hand. (2) Minimum Waiting Time to be Seen in the Office. Once within the office, waiting times are kept at a minimum. We recommend coming to the office about 15 minutes prior to the appointment time. In addition, it is suggested that pertinent intake forms be filled out in advance. These forms may be downloaded by simply clicking on the following link: PATIENT WELCOME & INTAKE FORMS. The forms will be downloaded if your computer has Adobe Reader installed. The forms may be printed out and filled out prior to your office visit. (3) Minimum Waiting Time for Reports to be Written and Sent to Referring Physicians and others. Reports are generally sent the same day to referring physicians or other individuals such as an individual's primary care provider (PCP). Reports may be sent via conventional mail, e-mail, or fax. HOW WE HANDLE INSURANCE CLAIMS We accept many insurance coverages for payment of audiological services. If necessary, a co-pay is paid at the time of the visit. Some insurance carriers require that a referral be made from your primary care provider. If this is the case, please be sure that the referral is in order before your appointment with us. We can help you arrange for your referral if you are unsure of the process. Hearing aids are sometimes covered in part or whole by insurance carriers or welfare funds. Most often, however, hearing aids are not covered by insurance. Our policy with regard to hearing aids is to have you reimbursed directly from your insurance company after payment to our office is received. We will assist you in all paperwork. Since there are many other companies not listed below, we ask that you tell us at the time of your initial call what company you subscribe. Very often, our services will be covered if enough time is allowed for us to call and verify the coverage type and policies of the carrier. It is also important to note that often, even within a given company, there are multiple rules and regulations for various plans. Therefore, please make sure that if a referral is required by your carrier, that the referral is in order prior to your visit.
